Application for Credit Download We have different application form depending on the type of account that you wish to set up. You will need Acrobat Reader to read these applications. Please select the type of account that best describes your needs: Use this application for individuals and families. Use this application for single proprietors, partnerships, LLC, Cooperatives, corporations (including nonprofit and not for profits). Applications are processed upon receipt. This generally takes one to three days. We will call or email you with our decision. Terms and Conditions 1. Invoices are net 30 (payment in full is expected each month); 2. Late payments will result in a $25.00 late payment fee; 3. Union Cab reserves the right to suspend charging priveledges or close the account at any time; 4. Account holders are expected to maintain accurate account information.
